The championship was first established on August 22, 2020, and the inaugural champion was Tyler Breeze.

The title (not recognized by WWE) was created by Xavier Woods as the "Mid-card title" for his UpUpDownDown YouTube game show and is defended under special rules: during LRLR title "matches", Woods provides the video game in question prior to the title match so that contestants have time to prepare for the game.

The LeftRightLeftRight Championship was retired on May 2, 2026, with the final champion being Tyler Breeze.
